Attention Saint Vincent Workers with Monetary Claims

All 1199SEIU members within the SVCMC network should have received one or
more Proof of Claim forms, asking you to identify all claims you have
against Saint Vincent Catholic Medical Centers institutions that are in
bankruptcy.

Please be advised that the Union is preparing a master Proof of Claim form
for all 1199SEIU bargaining unit members at SVCMC, for wage and benefits
claims relating to employment.

As long as members provide the Union with information about their claims, it
is unnecessary to file individual claims.

Please complete the questionnaire available here or from your Organizer or
Delegate.  Please return the form to your Organizer or Delegate as soon as
possible but no later than March 3, 2006.

These surveys will be forwarded immediately to 1199SEIU attorneys to prepare
the Claim form.  If you do not return the survey, the Union will not know
that claims you may have.
